The role

Whether you are an experienced nurse looking for a new challenge or newly qualified community nursing is for you!

We would be delighted to offer the opportunity to join our innovative, dedicated, and supportive team delivering outstanding care within the South-West Oxfordshire district nursing locality.

Our friendly teams take pride in supporting their team to deliver proactive, caring, safe and excellent standards of patient care to their local communities.

Our community nurses work autonomously, delivering high quality and evidence-based nursing care, see problem-solving as a positive challenge, and are creative in many varied and unpredictable clinical situations.

The District Nursing service in Oxfordshire is at the heart of developments in community care, as per the NHS Long Term Plan. A crucial part of the integrated community plans is recognition of the value of the District Nurse Specialist Practitioner Qualification for District Nursing Team Leaders, as advocated by The Queen's Nursing Institute, The King's Fund, and the RCN which we can support you to work towards.

The service currently operates 7 days a week between the core hours of 0800-1830. Shift patterns available are varied and can be discussed at interview with a flexible approach.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

What you'll do

  • Assess, plan, deliver and evaluate holistic, person-centred care programmes for patients with planned and urgent healthcare needs.
  • Manage and prioritise an allocated caseload, ensuring timely intervention, risk assessment, referral and discharge planning.
  • Deliver a wide range of community nursing interventions, recognising deterioration and responding appropriately to changing patient needs.
  • Work collaboratively with patients, carers, multidisciplinary teams and partner agencies to maximise independence, reduce hospital admissions and support safe discharge.
  • Provide leadership, supervision and support to staff, students and carers, contributing to workforce development and a positive learning environment.
  • Maintain high standards of clinical governance, record keeping, safeguarding and compliance with organisational policies, procedures and professional standards.
  • Contribute to service improvement, audit, quality initiatives and the effective use of resources to deliver safe, efficient and responsive community care.

We will support your progression through a District Nursing career progression framework, including advanced clinical practice and leadership training in this area. You will also receive regular 1:1 support and development and annual appraisal, regular group clinical supervision with your peers, and a comprehensive internal training programme around clinical competencies.

We hope that the advert has given you a clear understanding of the skills we are seeking and the opportunity at hand. You will need to use the "supporting statement" element of your application form to demonstrate your suitability for this role and you should refer to the job description, person specification and the guidance notes attached to this role to help you tailor your application.

The essential and desirable criteria will be used to shortlist for interview and you should ensure that you refer to these within your application to increase your chances of being selected for interview.

We are aware that some candidates may choose to use AI tools to support their application. We kindly remind applicants that submissions should remain an honest and accurate representation of their experience and must take care to ensure the use of AI tools does not generate an application that does not accurately reflect their knowledge, skills and values.
