This role involves providing specialist nursing care and advice to children with acute and long-term conditions, disabilities and complex health needs, including continuing care, respite, palliative and end-of-life care.
You will work with families, supporting care at home and advocating for the child's wishes and best interests. Working with health, education and social care partners, you will help deliver family-centred care.
The Oxfordshire Community Children's Nursing Team supports children and young people from birth to 18 years with complex health needs across the county. Based in Witney and supporting the West Oxfordshire locality, you will also travel across the county.
We offer induction, supervision and development opportunities.
Core service hours are 08:00-20:00, 7 days a week. Staff work shifts, including weekends and bank holidays.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
This role is subject to evaluation and duties may change.
What you'll do
- Deliver high-quality, child-centred nursing care, assessing, planning, implementing and evaluating individual programmes of care within the Community Children's Nursing Service.
- Support children and young people with complex, acute, life-limiting and end-of-life care needs, enabling them to receive care safely at home.
- Work in partnership with parents, carers and families to build confidence and competence in caring for their child and promoting positive outcomes.
- Collaborate with community teams, schools, hospitals and external agencies to ensure coordinated, seamless care for children and their families.
- Manage and prioritise an allocated caseload, acting as a clinical resource and providing specialist advice and support to colleagues and partner organisations.
- Provide supervision, mentorship and learning opportunities for junior staff and students, supporting their development and performance.
- Promote high standards of professional practice through effective record keeping, clinical governance, safeguarding, evidence-based care and advocacy for children's rights.
