Are you passionate about supporting older people to live fulfilling lives, maintain their independence and stay connected to the people and communities that matter to them?
We have an exciting opportunity for an Adult Social Care Practitioner to join our Communities North Team, within Sheffield City Council's Living and Ageing Well service.
Our team works predominantly with adults aged 65 and over who may have a range of physical health, mental health, care and support needs. About the role
As an Adult Social Care Practitioner, you will manage a varied caseload and undertake work in accordance with the Care Act 2014. This includes assessment, case management, care and support planning and reviews.
Your work will include:

- Working alongside people and carers to develop appropriate care and support plans.
- Completing reviews to ensure support continues to meet identified needs.
- Identifying, assessing and responding to risk and safeguarding concerns.
- Applying the principles of the Mental Capacity Act 2005, including assessing capacity and contributing to best-interest decision-making where required.
- Supporting people to build on their own strengths, networks and community resources to promote independence.
- Working collaboratively with health professionals, voluntary organisations and other partners.
- Providing information, advice and appropriate signposting.
- Maintaining clear and accurate records and evidencing the rationale for decisions.
- Undertaking home visits within communities across Sheffield.
About you
We are looking for someone who is person-centred, organised and able to build positive working relationships with adults, families, carers and other professionals.
You will have a practical understanding of the care and support needs that can arise from physical and/or mental health conditions and an understanding of relevant social care legislation, safeguarding and person-centred practice.
You should be able to:
- Assess care and support needs and identify and manage risk.
- Communicate confidently and sensitively with people, families and professionals.
- Work effectively as part of a multidisciplinary team.
- Manage and prioritise your own workload.
- Use your initiative and respond positively to changing situations.
- Take a creative and solution-focused approach to supporting people.
- Produce clear, evidence-based assessments and support plans.
- Promote independence, choice and least-restrictive approaches.
