The role

Role: Social Prescribing Link Work

Do you enjoy helping people build confidence, overcome challenges, and connect with opportunities in their local community?

We are looking for a compassionate and motivated Social Prescribing Link Worker to join our team. This is a rewarding opportunity to work alongside individuals, helping them identify what matters most to them and supporting them to improve their health, wellbeing, and quality of life.

Social prescribing is about people, not problems. It recognises that good health is influenced by many factors, including social connections, housing, finances, physical activity, confidence, and access to community support. As a Link Worker, you will help people discover solutions that work for them by connecting them with local groups, activities, and services.

What you'll do

  • Supporting South Ribble residents with issues important to them with an empathetic approach.
  • Working within a Wellbeing & Resilience service to provide effective signposting to immediate, and longer term support within the community.
  • Through partnership working, ensure accurate and timely interventions are delivered by the appropriate service or organisation.
  • Deliver strong team working to provide cover support for outreach drop-ins where needed.
  • Maintain accurate case notes and upload these to CharityLog in a timely manner.

How you'll work

Location: Civic Centre, West Paddock, Leyland, PR25 1DH

Contract: Full time (36.25 hours per week) / Permanent

Other requirements

  • This role requires some travel around our boroughs. You should have a valid driving licence or the equivalent mobility.
  • Successful applicants will be subject to a DBS check (applied and paid for by the Council).
