About us
Are you passionate about improving lives and empowering people to live independently? Whether you're an experienced social worker or just starting your journey, Hertfordshire County Council offers a range of exciting opportunities across our adult social care services.
We're transforming the way we work through our Connected Lives approach - a strengths-based model that puts people at the centre of everything we do. We focus on what matters most to individuals and their carers, helping them achieve their goals, stay connected to their communities, and live fulfilling lives.
About the role
As a Social Worker, you will play a key role in supporting adults, carers, and families to achieve positive outcomes and maintain their independence. Working within the discharge to assess model, you will help people transition safely from hospital, ensuring they receive the right support at the right time to maximise their wellbeing and reduce the need for ongoing services where appropriate.
Responsibilities include
- Undertaking strengths-based assessments and reviews in line with the Care Act 2014.
- Working with individuals, families, and carers to identify needs, strengths, and desired outcomes.
- Developing, implementing, and reviewing person-centred care and support plans.
- Providing advice, guidance, and support to help people maintain their independence and wellbeing.
- Managing a varied caseload and coordinating appropriate packages of care and support.
- Making referrals to partner agencies and working collaboratively within multidisciplinary teams.
- Assessing and managing risk, while ensuring safeguarding responsibilities are met.
- Maintaining accurate case records and preparing reports and documentation as required.
Hybrid working is available, with a minimum of two days per week based in the office.
About you
Essential
- A recognised Social Work qualification and registration with Social Work England.
- Successful completion of your ASYE and experience working within Adult Social Care.
- Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and safeguarding responsibilities.
- Experience of completing strengths-based assessments, support planning, and reviewing care and support arrangements.
- The ability to manage a varied caseload and balance competing priorities.
- Strong communication and relationship-building skills, with the ability to work effectively with individuals, families, carers, and partner agencies.
- A commitment to promoting independence and delivering person-centred, strengths-based practice in line with our Connected Lives approach.
- The ability to work collaboratively as part of a multidisciplinary team to achieve positive outcomes for residents.
- A full driving licence and access to a vehicle travel across the county as required.
We welcome applications from both experienced and newly qualified Social Workers who have completed their ASYE. Opportunities for newly qualified practitioners vary across our services and are considered on a vacancy-by-vacancy basis, taking into account the level of support and experience available within the team.
