Physical Health Nurse

• Registered General Nurse (RGN) with active NMC registration.
• Minimum 2 years' post-registration experience in a physical healthcare setting.
• Completed preceptorship programme.
• Knowledge of NMC guidelines and professional practice.
• Evidence of continued professional development.
• Strong communication and clinical assessment skills.

• Experience in physical healthcare within a hospital setting.
• Experience supporting patients with long-term conditions such as diabetes, hypertension or COPD.
• Experience with wound care, ECGs and phlebotomy.
• Experience in Infection Prevention and Control.
• Non-Medical Prescribing qualification.

Based in East Yorkshire, Priory Hospital Market Weighton is a level 2 rehabilitation hospital for adult males, offering bespoke and flexible therapeutic programme with therapies focused on stabilisation, skill acquisition, personal development, community integration, and developing competence and confidence in living skills.

• Lead on patients' physical healthcare assessments, care planning and ongoing reviews.
• Provide specialist advice on areas such as diabetes, hypertension, COPD and wound care.
• Lead Infection Prevention & Control across the site, including audits and outbreak management.
• Deliver physical healthcare interventions including ECGs, phlebotomy and wound care.
• Liaise with primary care services to coordinate patients' ongoing healthcare needs.
• Support and train nursing staff in physical healthcare, observations and clinical assessments.
• Lead health promotion and smoking cessation initiatives.
