The role

Our Adult Social Care Service provides support across a range of teams, including Initial Assessment, Locality, Hospital Discharge, Specialist Learning Disability, and Autism Services.

We currently have a Social Worker vacancy within our Preparation for Adulthood and Autism Team. This role supports young people with disabilities and additional needs as they transition from Children's Services into Adult Social Care, as well as adults with autism to access appropriate care, support, and opportunities that promote independence and wellbeing.

The post holder will work as part of the Transition and Autism Service and in close partnership with the Children with Disabilities Team, Leaving Care Team, Special Educational Needs (SEN) Services, Health Professionals, families, carers, and other key stakeholders to ensure effective transition planning and positive outcomes for young people and adults with autism.

What you'll do

  • Undertake timely and responsive Care Act Assessments, reviews, and reassessments, ensuring a person-centred approach that promotes choice, independence, wellbeing, and safety.
  • Provide effective case management for young people transitioning into Adult Social Care and adults with autism requiring ongoing support.
  • Contribute to and review Education, Health and Care Plans (EHCPs), Health Action Plans, transition plans, and other person-centred support plans.
  • Support individuals to access opportunities for employment, education, training, volunteering, and community participation.
  • Participate in Continuing Healthcare (CHC) Assessments and multi-disciplinary meetings where appropriate.
  • Work collaboratively with statutory, voluntary, health, housing, and independent sector organisations to coordinate support and achieve positive outcomes.
  • Undertake assessments and interventions under the Mental Capacity Act 2005, including capacity assessments, Best Interest decision-making, and applications relating to Community Deprivation of Liberty Safeguards where required.
  • Manage safeguarding concerns and undertake Section 42 Safeguarding Enquiries, risk assessments, protection planning, and associated interventions.
  • Complete person-centred planning and support individuals to achieve outcomes that maximise independence and quality of life.
  • Make referrals to appropriate services, including supported living, residential placements, respite services, day opportunities, community support services, and specialist autism provision.
  • Carry out strengths-based assessments and reviews, ensuring that the voice of the individual and their family/carers is central to decision-making.
  • Participate in the team's duty system, responding to urgent issues and risks in a timely and proportionate manner.
  • Contribute to service development, quality improvement initiatives, and the sharing of professional knowledge and best practice across the service.
  • Represent the service at internal and external meetings, panels, reviews, and multi-agency forums as required.
